for Minor Youth Coaches

IMPORTANT: Please read below before continuing

• Must be at least 16 years old
• Must complete all online trainings as required by HYSA for each seasonal year
• Expires on the day before 18th birthday.
• $5 fee (one time fee only)

Upload a headshot photo (individual pictures only) that will be printed on your pass.  
*No photos wearing hats or sunglasses; no shirtless photos

Face directly at camera; sufficient lighting on facial area recommended

 Unacceptable photos will result in a delay of your pass. 
All completed passes will be mailed to the address in the Affinity System (except Kauai)

 Minor Youth Coaches must have an adult risk management approved coach present at all times.

*Kauai passes will be mailed to the Kauai league registrars)
